Overhauled our Concurrency Limit Mechanism

The concurrency mechanism keeps track of how many recordings can be processed simultaneously for each account. It only kicks in when the processing server is under high load.

We’ve made a slew of changes in how it works behind the scenes to make it more robust in the face of external errors.
